Who do I contact if I have billing or insurance questions?

Please contact our office with any billing or insurance questions we will be happy to assist you.

How far in advance should I arrive for my appointment?

Patient’s should arrive 30-45 minutes prior to their appointment to fill out any forms necessary. All forms are available online if you wish to fill them out ahead of time.

What item’s should I bring to my appointment?

It is helpful that you bring any previous medical records and a complete list of medications with dosage information. In addition, name of previous medical providers and their contact information.

Will I be provided with a reminder call prior to my appointment?

Yes, as a courtesy to our patients we will call to remind you of your appointment the day prior.

Why do I need to schedule a follow-up appointment?

In general, a follow-up appointment may be required to provide proper medical treatment and assessment of your symptoms or review prescriptions prescribed.

Where do I go if I need lab work and or other testing done?

Facilities available for lab tests or diagnostic tests are listed below. Be sure to verify with your insurance carrier what facilities they are contracted with before obtaining tests. Clicking in the name of the lab will take you to a map to help you find your way.

How long before my lab tests or test results are available?

Test results are usually available within 2 days. Please keep in mind some tests or blood tests may require a longer time-frame or may need to be sent out to another facility for processing.

What steps do I follow if I need a refill of my prescription?

If you require a refill of a prescription please contact your pharmacy directly and request a refill request faxed to our office. If your refill request is for a narcotic, you must make an appointment to be seen in the office.
